If a vendor is not in the FMS system, this form is needed to add them. This form is also used to update contact information such as an address or name change.
This form is available for download at: http://fms.k12.hi.us/forms/
Vendors must provide an IRS Form W-9 in order to fill out this information. The W-9 form is also required for the clerk to input it into the system.
1. Enable Editing
After opening the document, it may open in Protected View. Click on Enable Editing to Continue
