Creating/Updating a Vendor

If a vendor is not in the FMS system, this form is needed to add them. This form is also used to update contact information such as an address or name change.

Vendors must provide an IRS Form W-9 in order to fill out this information. The W-9 form is also required for the clerk to input it into the system.

1. Enable Editing

After opening the document, it may open in Protected View. Click on Enable Editing to Continue

2. New or Change

Check mark either New or Change.

2.1. (If Change) Add Vendor Number

If you are making a change to a vendor, add the vendor number in the field shown above. The vendor number is available when selecting a vendor in Lotus Notes.
